Transitional Living

Transitional Services is about meeting the client where they are and assisting them to where they want to go. 43 North Iowa has personal and flexible programming to support our clients to engage in life, to achieve emotional stability, and move to become self-sufficient. Our staff in various programs provide guidance and support for people to make choices to navigate their experiences healthily.

Support Community Living & Community Support Services

 

Supported Community Living Program (SCL): In our SCL program, clients are partners with staff for assistance in maintaining their independent living situation. The SCL team assists clients anywhere from daily up to five times a week between an hour or two. Clients can receive support in medication management, daily life skills, and other areas of need.

Habilitation Home Program (HAB)

Habilitation Home Program (HAB): In our Hab program, clients are supported by the Hab team in a shared space environment from 6 to 10 hours a day. The HAB team helps the client in accessing the community and increasing the client’s ability to navigate their life independently. The client can seek support with medication management, access to medical and mental health care, daily life skills, and other areas of need.

Residential Care Facility (RCF)

Residential Care Facility (RCF): In our RCF program, clients are supported by the RCF team in the facility 24 hours a day. The team helps clients manage their mental health and daily struggles through groups, activities, and individual sessions with staff. The client builds coping skills, life skills by working with their team on medication management, access to medical and mental health care, and daily life skills. Clients and staff also seeking additional assistance to move into a more independent environment.

The goal of every program at 43 North Iowa is for the client to achieve the most significant independence they feel they can maintain.
